Summary

Skeletal muscle index (SMI) may help identify patients with acute mesenteric ischemia (AMI) at higher risk of mortality. Lower SMI was associated with increased 30-day mortality in this patient cohort.

Background:

  • Body composition, including low skeletal muscle mass (LSMM) and adipose tissue (SAT, VAT), is measurable via cross-sectional imaging.
  • Previous studies indicate prognostic relevance of body composition in various diseases.

Purpose of the Study:

  • To investigate associations between body composition parameters and mortality in patients with clinically suspected acute mesenteric ischemia (AMI).

Main Methods:

  • Retrospective analysis of 137 patients with suspected AMI (2016-2020).
  • Preoperative abdominal CT scans used to calculate LSMM, VAT, and SAT.
  • Skeletal muscle index (SMI) derived from CT imaging.

Main Results:

  • High 30-day mortality rate (68.6%) observed in the cohort.
  • Lower SMI was significantly associated with increased mortality (37.5 ± 12.4 cm²/m² in non-survivors vs. 44.1 ± 13.9 cm²/m² in survivors, p=0.01).
  • No significant associations found between SMI, SAT, or VAT and mortality in days; a trend for visceral obesity was noted (HR: 0.62, p=0.07).

Conclusions:

  • SMI may serve as a valuable CT-based parameter for discriminating between survivors and non-survivors in AMI.
  • Further research is required to fully understand the relationship between body composition and survival in AMI patients.
