    Diabetic nephropathy and immune-complex glomerulonephritis can occur simultaneously. This study highlights 3 cases where infections preceded or coincided with kidney damage, showing potential for improved renal function with treatment.

    Background:

    • Diabetic nephropathy is a common complication of diabetes mellitus.
    • Immune-complex mediated glomerulonephritis is an inflammatory kidney disease.
    • Simultaneous occurrence of both conditions is rare but clinically significant.

    Observation:

    • Three patients presented with proteinuria and hematuria.
    • Renal manifestations were linked to preceding or concurrent infections.
    • Kidney biopsies revealed features of both diabetic nephropathy and immune-complex glomerulonephritis.

    Findings:

    • Immunofluorescence microscopy showed a dual pattern of IgG and C3 deposits: linear and granular.
    • Two patients demonstrated improvement in renal function and urinary findings after follow-up.
    • The co-existence of these conditions presents a complex diagnostic and therapeutic challenge.

    Implications:

    • This study underscores the importance of considering co-existing renal pathologies in diabetic patients.
    • Infectious triggers may play a role in the onset of immune-complex glomerulonephritis in diabetic individuals.
    • Early diagnosis and management may lead to better renal outcomes in such complex cases.
