Frequent straining and chronic stimulant laxative use predict inadequate bowel preparation for colonoscopy. Severely constipated patients with these habits may need intensified preparation regimens.
Area of Science:
- Gastroenterology
- Endoscopy
- Clinical Research
Background:
- Inadequate bowel preparation (BP) compromises colonoscopy efficacy and quality.
- Constipation is a known predictor of poor BP, but specific bowel habit details remain underexplored.
Purpose of the Study:
- To investigate the association between specific bowel habits, constipation severity, and inadequate BP.
- To identify predictors of inadequate BP in patients undergoing colonoscopy.
Main Methods:
- Prospective observational study of 1054 patients undergoing colonoscopy.
- Assessment of bowel habits, constipation severity (constipation scoring system), and BP quality (Boston Bowel Preparation Scale).
- Multivariate logistic regression analysis to identify predictors of inadequate BP.
Main Results:
- 10% of patients (105/1054) had inadequate BP.
- Increased constipation severity correlated with higher risk of inadequate BP (P=0.01).
- Frequent straining (OR 2.09) and chronic stimulant laxative use (OR 2.57) were significant predictors of inadequate BP.
Conclusions:
- Frequent straining and chronic stimulant laxative use are key predictors of inadequate BP.
- Consider intensified bowel preparation protocols for constipated patients exhibiting straining and chronic stimulant laxative use.
