Intermittent ex vivo lung perfusion (EVLP) improved lung graft condition compared to cold static preservation. While the second EVLP showed slight functional decline, it maintained better graft quality, suggesting potential for extended lung preservation.

Area of Science:

  • Transplantation research
  • Organ preservation science

Background:

  • Ex vivo lung perfusion (EVLP) extends lung preservation time and donor pool beyond cold static preservation.
  • Hypothesized that repeated EVLP would better maintain lung grafts.

Purpose of the Study:

  • To evaluate the efficacy of intermittent ex vivo lung perfusion (EVLP) in maintaining lung graft quality compared to standard cold static preservation.

Main Methods:

  • Ten pig lungs were divided into two groups: control (16h cold ischemic time + 2h EVLP) and intermittent EVLP (4h cold ischemic time + 2h EVLP + 10h cold ischemic time + 2h EVLP).
  • Lungs were assessed for transplant suitability after EVLP.

Main Results:

  • Intermittent EVLP showed improved oxygenation, reduced edema, and better mitochondrial preservation than the control, despite increased cytokines.

  • The second EVLP session resulted in lower dynamic compliance and higher extravascular lung water compared to the first EVLP session.
  • Transplant suitability was 100% for the first EVLP, 60% for the second EVLP, and 0% for the control group.
  • Conclusions:

    • Intermittent EVLP offers superior lung graft condition compared to cold static preservation.
    • While the second EVLP session showed a slight functional decrease, intermittent EVLP shows promise for prolonging lung preservation.
    • Further research may optimize intermittent EVLP protocols to maximize graft viability.
