    Summary

    Non-steroidal anti-inflammatory drugs (NSAIDs) can cause acute proctocolitis, a rectal inflammation. Reintroduction of NSAIDs in patients with resolved symptoms led to rapid symptom recurrence, confirming a causal link.

    Background:

    • Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for pain and inflammation.
    • NSAID-induced enterocolitis is a recognized but uncommon adverse effect.

    Observation:

    • Four patients presented with acute proctocolitis.
    • NSAIDs implicated included flufenamic acid, mefenamic acid, naproxen, and ibuprofen.

    Findings:

    • Three out of four patients experienced a rapid relapse of proctocolitis upon rechallenge with the implicated NSAID.
    • This strongly suggests a direct causal relationship between NSAID use and acute proctocolitis.

    Implications:

    • Clinicians should consider NSAIDs in the differential diagnosis of acute proctocolitis.
    • Avoiding implicated NSAIDs is crucial for managing and preventing recurrent episodes.
