Background:
- Diffuse large B-cell lymphoma (DLBCL) is the most common Non-Hodgkin Lymphoma (NHL).
- Primary sinonasal NHL is exceptionally rare, presenting diagnostic and therapeutic hurdles.
- DLBCL can affect virtually any organ, including rare presentations in the sinonasal tract.
Observation:
- A 49-year-old woman presented with diplopia and right superior eyelid swelling.
- Clinical, radiological, and histological findings confirmed DLBCL of the right frontal sinus.
- The tumor invaded subcutaneous soft tissues and the intracranial frontal region.
Findings:
- The patient received multiple chemotherapy regimens (MATRIX, R-DA-EPOCH) and CAR-T therapy.
- Despite aggressive treatment, the patient's condition was refractory.
- The patient died 11 months post-diagnosis.
Implications:
- DLBCL of the frontal sinus poses significant diagnostic challenges, often mimicking other sinonasal pathologies.
- Delayed diagnosis in sinonasal DLBCL can impede timely and effective treatment initiation.
- This case highlights the critical need for heightened awareness and diagnostic vigilance for rare sinonasal malignancies.
