Background:

Pulmonary mucoepidermoid carcinoma (PMEC) is a rare lung malignancy.
The association between PMEC and paraneoplastic syndromes is not well-documented.
Multiple paraneoplastic syndromes in a single lung cancer patient are uncommon.

Observation:

A patient with metastatic high-grade PMEC presented with multiple paraneoplastic syndromes.
The observed syndromes included humoral hypercalcemia of malignancy, ectopic ACTH syndrome, and paraneoplastic leukemoid reaction.

Findings:

This case details a rare presentation of metastatic high-grade PMEC.
The patient exhibited three distinct paraneoplastic syndromes concurrently.
This presentation underscores the complex and varied clinical manifestations of PMEC.

Implications:

Increased awareness of PMEC's potential to cause multiple paraneoplastic syndromes is crucial for diagnosis.
This case may prompt further research into the mechanisms linking PMEC and paraneoplastic phenomena.
Recognizing these associations can improve patient management and outcomes for rare lung cancers.
