Summary

Continuous positive airway pressure (CPAP) for obstructive sleep apnea (OSA) did not significantly reduce cardiovascular events overall. However, good adherence to CPAP therapy was linked to a lower risk of recurrent cardiovascular events.

Background:

  • The role of continuous positive airway pressure (CPAP) in preventing secondary cardiovascular disease is a subject of ongoing debate.
  • Obstructive sleep apnea (OSA) is a common condition often associated with increased cardiovascular risk.

Purpose of the Study:

  • To evaluate the impact of CPAP treatment for OSA on the incidence of adverse cardiovascular events.
  • To determine the effectiveness of CPAP in secondary cardiovascular prevention through a meta-analysis of randomized clinical trials.

Main Methods:

  • A systematic search of multiple databases (PubMed, EMBASE, CENTRAL, ClinicalTrials.gov) was conducted up to June 22, 2023.
  • Individual participant data (IPD) from relevant randomized clinical trials were analyzed using one-stage and two-stage meta-analyses with mixed-effect Cox regression models.
  • On-treatment analyses with marginal structural models were employed to assess the effect of CPAP adherence.

Main Results:

  • The overall analysis of 4186 participants showed no significant difference in major adverse cardiac and cerebrovascular events (MACCEs) between CPAP and usual care groups (HR, 1.01; 95% CI, 0.87-1.17).
  • However, an on-treatment analysis revealed a significantly reduced risk of MACCEs in patients with good CPAP adherence (≥4 hours/day) (HR, 0.69; 95% CI, 0.52-0.92).

Conclusions:

  • While overall CPAP use did not demonstrate a significant benefit in preventing recurrent cardiovascular events, good adherence to CPAP therapy is associated with a reduced risk.
  • Treatment adherence is a critical factor for effective secondary cardiovascular prevention in patients with OSA.
