Summary

Preoperative planning software accurately measures cochlear implant (CI) skin flap thickness. Thicker skin flaps in CI patients correlate with patient age and require stronger magnets for optimal device function.

Area of Science:

  • Otolaryngology
  • Medical Imaging
  • Biomedical Engineering

Background:

  • Preoperative assessment of skin flap thickness is crucial for cochlear implant (CI) surgery.
  • Determining the appropriate magnet strength is essential for CI device management.
  • Accurate pre-planning aids in optimizing surgical outcomes and device performance.

Purpose of the Study:

  • To evaluate the efficacy of OTOPLAN®, a tablet-based otological preplanning tool, in measuring skin flap thickness.
  • To investigate the correlation between skin flap thickness and the required magnet strength in CI patients.

Main Methods:

  • Fifty-seven temporal bone CT datasets from CI patients were analyzed.
  • OTOPLAN® software was utilized to measure skin flap thickness in axial and coronal views.
  • Measurements were standardized using the superior aspect of the pinna as a reference point.

Main Results:

  • The average skin flap thickness was 4.5 ± 1.2 mm.
  • A strong inter-rater reliability (Cronbach's alpha = 0.90) was achieved.
  • Significant positive correlations were found between skin flap thickness and patient age (r = 0.69, p = 0.002) and magnet strength (r = 0.82, p < 0.0001).

Conclusions:

  • OTOPLAN® is a reliable and efficient tool for measuring skin flap thickness in CI surgery planning.
  • Patient age and required magnet strength are positively correlated with skin flap thickness.
  • These findings support the use of OTOPLAN® for improved preoperative planning in CI procedures.
