The Expanded Cutting Alignment Scoring Tool (E-CAST) shows good validity for assessing lower extremity alignment in female athletes during sidestep cuts, with strong predictive accuracy for most movement errors. This tool can aid in identifying injury risk factors in sports.

Background:

  • The Expanded Cutting Alignment Scoring Tool (E-CAST) is a reliable method for assessing lower extremity alignment during sidestep cuts.
  • However, its validity in capturing movement errors has not been established.
  • This study aimed to determine the concurrent validity of the E-CAST.

Purpose of the Study:

  • To assess the concurrent validity of the E-CAST.
  • To compare visually identified movement errors from 2D video analysis with 3D biomechanical data.
  • To evaluate the E-CAST's ability to identify injury risk factors in female athletes.

Main Methods:

  • Sixty female athletes (mean age 14.1 years) performed a 45-degree sidestep cut.
  • Simultaneous 2D video and 3D motion capture were used.
  • A clinician scored 2D videos using E-CAST criteria; 3D data included joint angles and moments. Receiver operating characteristic (ROC) curve analyses were performed.

Main Results:

  • ROC analyses demonstrated acceptable to outstanding Area Under the Curve (AUC) values (0.67-0.91) for most E-CAST items.
  • Sensitivity and specificity ranged from 70-85% and 55-89%, respectively.
  • The E-CAST showed high accuracy in identifying biomechanical risk factors, except for static knee valgus.

Conclusions:

  • The E-CAST demonstrates good concurrent validity for assessing lower extremity alignment during sidestep cuts.
  • The tool is effective in identifying movement errors associated with injury risk.
  • Further refinement may be needed for static knee valgus assessment.
