    Background:

    • Suicidal patients' motivations are often ambiguous.
    • The changeability of these motivations is understudied.

    Purpose of the Study:

    • To investigate the changeability of suicidal motivations in patients who attempted suicide by paraquat overdose.
    • To assess the reliability of stated suicidal intent over time.

    Main Methods:

    • Twenty-six paraquat overdose patients were interviewed regarding their motivations at multiple time points.
    • Motivations were assessed at admission, 24 hours, 3, 7, and 10 days post-admission.

    Main Results:

    • Initially, 25 patients wished to die; however, all 14 survivors desired to live by hospital discharge.
    • Six of the 12 deceased patients no longer wished to die before their death.
    • Three survivors died by suicide within one year.

    Conclusions:

    • Suicidal patients' stated motivations are highly changeable.
    • Thorough assessment of all patient factors, not just stated intent, is crucial for risk determination.
    • Paraquat ingestion is a highly lethal suicide attempt method.
