Summary

A rare case of a giant gallbladder, measuring 20 cm and containing over 30 stones, was successfully treated with laparoscopic cholecystectomy. This surgical approach offers a viable option for managing such rare gallbladder conditions.

Background:

Overdistended gallbladders are typically linked to bile duct obstruction from malignancy or gallstones.
Grossly distended gallbladders (>14 cm) without identifiable pathology are exceptionally rare.
This case highlights a unique presentation of a giant gallbladder.

Observation:

A 46-year-old female presented with acute right lower quadrant pain.
Initial diagnoses included liver and choledochal cysts.
Magnetic resonance cholangiopancreatography revealed a giant gallbladder (20 cm x 7 cm) with >30 stones.

Findings:

Laparoscopic cholecystectomy was successfully performed after gallbladder decompression.
The patient experienced complete pain resolution post-operation.
Three-year follow-up showed no recurrence, with normal liver function and imaging.

Implications:

Laparoscopic cholecystectomy is an effective treatment for giant gallbladders.
This case contributes to understanding rare gallbladder pathologies.
Successful management of this rare condition may inform future treatment strategies.
