Summary

Total fertilization failure in IVF is linked to a lower number of retrieved oocytes, particularly in cases without male factor infertility. Retrieving fewer than 5 oocytes significantly increases the risk of IVF failure.

Background:

  • Total fertilization failure (TFF) in IVF presents a significant challenge, especially when male factors are not the primary cause.
  • Identifying female-specific parameters associated with TFF is crucial for improving IVF success rates.

Purpose of the Study:

  • To identify male and female parameters linked to TFF in IVF among patients with non-masculine indications.
  • To determine the predictive value of oocyte yield on IVF failure.

Main Methods:

  • A case-control study analyzed 154 TFF cases and 475 controls from 2010-2019.
  • Evaluated female and male infertility parameters, excluding cases with abnormal semen analysis.
  • Logistic regression analysis identified significant predictors of TFF.

Main Results:

  • Female and male age, anti-Müllerian hormone (AMH), semen volume, and initial sperm motility were not significantly different between groups.
  • Significantly fewer oocytes were collected in the TFF group (median 2 vs. 5).
  • A lower number of collected oocytes (fewer than 5) was significantly associated with TFF (OR -1.37).

Conclusions:

  • A diminished follicular response, indicated by a lower oocyte yield, is the primary factor associated with IVF failure in non-masculine infertility.
  • Oocyte number is a critical parameter for predicting IVF success in the absence of male factor infertility.
