Phases of Wound Repair01:28

Phases of Wound Repair

Following injury, the integrity of the injured tissues must be reestablished. For example, in skin tissue, wound repair involves coordination among resident skin cells, blood mononuclear cells, extracellular matrix, growth factors, and cytokines to complete the healing cascade.
Formation of Blood Clot
In case of deep injuries, trauma to blood vessels results in blood loss. In the meantime, phospholipids released from the ruptured endothelial cellular membrane are converted into arachidonic...

Fractures: Bone Repair

Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the procedure...

Summary

Propeller perforator flaps offer a viable solution for immediate reconstruction of Gustilo IIIB open fractures, a technique not previously reported in acute trauma settings. This method shows promising results with no major complications in initial patient cases.

Area of Science:

  • Orthopedic Surgery
  • Reconstructive Surgery
  • Trauma Management

Background:

  • Open bone fractures, particularly Gustilo IIIB, pose significant risks to limb and life.
  • Reconstruction of soft tissue defects in the lower extremity often requires advanced techniques.
  • Propeller perforator flaps present a potential reconstructive option for complex fractures.

Purpose of the Study:

  • To evaluate the efficacy of immediate propeller perforator flap reconstruction for Gustilo IIIB open fractures.
  • To report the initial experience with this technique in an acute trauma setting.
  • To assess the safety and outcomes of propeller perforator flaps in this context.

Main Methods:

  • Retrospective review of five patients with Gustilo IIIB open fractures treated between 2019 and 2022.
  • Immediate reconstruction using propeller perforator flaps.
  • Fracture reduction, fixation, and flap coverage performed in the acute setting.

Main Results:

  • Mean operating time was 106 minutes.
  • Successful flap coverage for defects ranging from 5-7 cm in length and 3-4 cm in width.
  • No major complications or need for surgical revision, except for superficial necrosis in one smoker.

Conclusions:

  • Propeller perforator flaps are a viable reconstructive option for immediate management of Gustilo IIIB open fractures.
  • This technique has not been previously reported for acute trauma reconstruction.
  • The study demonstrates promising initial outcomes for this approach in trauma care.
