    Background:

    • Herpes gestationis (HG) is an autoimmune blistering disease occurring during pregnancy.
    • Its precise immunopathological mechanisms require further elucidation.

    Purpose of the Study:

    • To investigate the immunopathological and microscopic changes in skin biopsies from patients with herpes gestationis.
    • To correlate clinical presentation with histological and immunofluorescence findings.

    Main Methods:

    • Skin biopsies from involved and uninvolved skin of eleven HG patients.
    • Direct immunofluorescence (DIF) for immune deposits.
    • Immuno-electron microscopy (IEM) for ultrastructural localization of immune deposits.
    • Light microscopy (LM) for histological changes.

    Main Results:

    • Direct immunofluorescence revealed C3 and/or IgG deposition at the basement membrane zone in 9/11 involved and 5/11 uninvolved skin samples.
    • Immuno-electron microscopy showed in vivo IgG deposition at the basal cell membrane extending into the lamina lucida.
    • Light microscopy demonstrated papillary dermal edema, perivascular inflammation, epidermal spongiosis, basal cell necrosis, and sub-epidermal clefts/bullae.

    Conclusions:

    • Herpes gestationis is characterized by immune complex deposition (C3, IgG) at the dermal-epidermal junction.
    • Ultrastructural analysis confirms IgG deposition on basal cells.
    • Histopathological findings include significant basal cell damage and blister formation, consistent with autoimmune blistering.
