Summary

Kidney transplants from donors on extracorporeal membrane oxygenation (ECMO) can expand the donor pool. While VA-ECMO kidneys show comparable survival to standard donors, VV-ECMO kidneys have higher graft loss risks.

Background:

Purpose of the Study:

  • To evaluate the safety and efficacy of using renal allografts from ECMO-supported donors for kidney transplantation.
  • To compare transplant outcomes between ECMO donors (VA-ECMO and VV-ECMO) and non-ECMO donors stratified by Kidney Donor Profile Index (KDPI).

  • VV-ECMO, but not VA-ECMO, was associated with increased risk of graft loss (HR 2.18).
  • High KDPI, older donor age, higher BMI, hypertension, and diabetes were risk factors in ECMO donors.

Conclusions:

  • Kidney transplantation from carefully selected ECMO donors can safely increase the available donor pool.
  • VA-ECMO donors represent a viable option for kidney transplantation.
  • Further research is needed to establish optimal practices for utilizing kidneys from ECMO-supported donors, particularly VV-ECMO donors.
