Summary

Prophylactic antibiotic beads may help reduce infection risk in high-risk breast reconstruction patients. This pilot study suggests absorbable antibiotic beads normalize infection rates in these cases.

Background:

Purpose of the Study:

  • To evaluate the efficacy of prophylactic absorbable calcium sulfate antibiotic beads.
  • To assess infection rates in high-risk, implant-based, prepectoral breast reconstruction.
  • To explore a novel application of antibiotic beads in plastic surgery.

Main Methods:

  • Retrospective review of 148 patients (2019-2022) undergoing prepectoral breast reconstruction.
  • Comparison between patients receiving antibiotic beads (n=15) and a control group (n=133).
  • Primary outcome: postoperative surgical site infection at 90 days.

Main Results:

  • Antibiotic bead group had a higher history of infection (66.7% vs 0%, P < 0.01).
  • Infection rates were 3.2% (bead group) vs 7.6% (control group), not statistically significant.
  • Antibiotic beads appeared to normalize infection incidence in high-risk patients.

Conclusions:

  • Absorbable antibiotic beads may be a viable option for infection prophylaxis in high-risk breast reconstruction.
  • This pilot study supports further investigation into antibiotic beads for prepectoral implant placement.
  • Novel use of prophylactic absorbable antibiotic beads in breast reconstruction is presented.
