    Summary

Anterior clear spaces, a finding in keratoconus (an eye condition), were observed in 38% of patients. Microscopic examination confirmed these spaces are linked to breaks in Bowman

    Area of Science:

    • Ophthalmology
    • Corneal Diseases
    • Microscopy

    Background:

    • Keratoconus is a progressive eye condition characterized by thinning and bulging of the cornea.
    • Clinical presentation of keratoconus can vary, necessitating comprehensive diagnostic approaches.

    Purpose of the Study:

    • To prospectively investigate the occurrence and histological basis of anterior clear spaces in keratoconus patients.
    • To determine if anterior clear spaces represent a distinct clinical sign associated with keratoconus.

    Main Methods:

    • Prospective observational study involving slit-lamp examinations of consecutive keratoconus patients.
    • Histopathological analysis of corneal buttons from affected eyes using light and electron microscopy.

    Main Results:

    • Anterior clear spaces were identified in 38% of 69 consecutive keratoconus eyes.
    • Microscopic examination of two corneal buttons revealed breaks in Bowman's layer corresponding to the anterior clear spaces.

    Conclusions:

    • Anterior clear spaces are a notable clinical finding in a significant proportion of keratoconus cases.
    • The presence of breaks in Bowman's layer provides a histological explanation for anterior clear spaces in keratoconus.
    • Anterior clear spaces should be recognized as part of the clinical spectrum of keratoconus.
