Purpose of the Study:

  • To enhance adherence to early peanut introduction guidelines in primary care.
  • To implement a bundle of clinical decision support (CDS) tools within an electronic medical record (EMR).

Main Methods:

  • Quality improvement methodology was employed.
  • Standardized work protocols and CDS tools were developed for 4, 6, and 9-month well-child check (WCC) encounters.
  • Plan-do-study-act cycles and statistical process control charts were used for analysis.

Main Results:

  • EMR documentation of EPI guidance increased from 13.9% to 83.5% over 12 months.
  • Provider adoption of CDS tools rose to 73%, and handout distribution to 54.1%.
  • Caregiver-reported peanut ingestion increased from 0% to 34.6% without affecting vaccination rates or visit times.

Conclusions:

  • Quality improvement initiatives effectively increased EPI guideline adherence and peanut ingestion.
  • Bundled CDS tools and EMR standardization show promise for widespread peanut allergy prevention.
  • Routine WCC encounters are a viable setting for promoting EPI.
