    Summary

    Measuring serum immunoreactive parathyroid hormone (iPTH) is a sensitive diagnostic tool for primary hyperparathyroidism before surgery. However, postoperative iPTH levels are not reliable indicators of persistent disease.

    Background:

    • The diagnostic utility of serum immunoreactive parathyroid hormone (iPTH) in primary hyperparathyroidism is debated.
    • The clinical significance of elevated postoperative serum iPTH levels remains unclear.

    Purpose of the Study:

    • To evaluate the clinical value of serum iPTH measurements in diagnosing primary hyperparathyroidism.
    • To assess the significance of elevated postoperative serum iPTH levels after parathyroidectomy.

    Main Methods:

    • Studied 141 consecutive patients with primary hyperparathyroidism.
    • Measured serum iPTH using a mid-region-specific radioimmunoassay before and after parathyroidectomy.
    • Correlated iPTH levels with serum calcium, blood urea nitrogen, serum creatinine, alkaline phosphatase, and tumor size.

    Main Results:

    • Preoperative serum iPTH was elevated in 88% of patients, with others showing inappropriate levels relative to calcium.
    • Preoperative iPTH correlated positively with serum calcium and parathyroid tumor size.
    • Postoperative iPTH elevation occurred in up to 40% and was linked to higher preoperative BUN, creatinine, alkaline phosphatase, and larger tumors.
    • Elevated postoperative iPTH without hypercalcemia did not signify parathyroidectomy failure.
    • Negative parathyroid exploration and postoperative hypercalcemia predicted persistent hyperparathyroidism.

    Conclusions:

    • Preoperative serum iPTH measurement is a highly sensitive diagnostic test for primary hyperparathyroidism.
    • Postoperative serum iPTH measurement is not a reliable predictor of persistent or recurrent hyperparathyroidism.
