Prosthesis Emergence Angle Influences Marginal Bone Level for External Connection Implants: A Cross-Sectional Study.

    Summary

Longer implant length and greater emergence angle (EA) were linked to increased marginal bone loss (MBL) around bone-level implants. Convex crown profiles also correlated with higher distal MBL, impacting implant longevity.

    Area of Science:

    • Dental Implantology
    • Prosthodontics
    • Periodontology

    Background:

    • Bone-level implants with external hex connections are widely used.
    • Understanding factors influencing marginal bone level (MBL) changes is crucial for long-term implant success.
    • Prosthetic features may play a significant role in peri-implant bone health.

    Purpose of the Study:

    • To evaluate the impact of prosthetic features on marginal bone level changes.
    • To investigate the relationship between emergence angle (EA), emergence profile (EP), crown-implant ratio (CIR), and abutment height with marginal bone loss (MBL).
    • To analyze these factors around bone-level implants with external hex connections.

    Main Methods:

    • Retrospective analysis of 166 implants in 100 patients with cemented crowns.
    • Radiographic evaluation of prosthetic features: EA, EP, CIR, and abutment height.
    • Measurement of MBL from intraoral radiographs at baseline and follow-up (≥1 year).

    Main Results:

    • Mean follow-up was 43.94 months. Mean MBL was 0.19 mm (mesial) and 0.20 mm (distal).
    • Significant positive associations found between MBL and implant length (P < .005) and EA (P < .05).
    • Convex crown profiles showed higher distal MBL (P = .025) compared to concave/straight profiles.

    Conclusions:

    • Implant length and EA are significant factors associated with MBL around bone-level implants.
    • Crown profile influences MBL, with convex profiles potentially leading to greater bone loss.
    • These findings highlight the importance of prosthetic design in maintaining peri-implant bone levels.
