Ovaries

The ovaries are roughly the size of almonds and measure approximately 2 to 3 centimeters in length. These paired structures are situated within the pelvic region and are anchored by the mesovarium—a peritoneal extension that also connects them to the wider structure of the broad ligament. Background:

Undescended ovaries are rare and typically asymptomatic.
Infertility cases may require intervention when ovarian location impedes assisted reproductive technology (ART), such as transvaginal egg retrieval.

Observation:

A novel laparoscopic surgical approach was developed for mobilizing and repositioning undescended ovaries.
The technique was applied to a nulligravid woman with a history of infertility and prior failed ovarian surgery.

Findings:

The surgical intervention resulted in successful oophoropexy, with ovaries located posterior to the uterus.
Postoperative recovery was marked by minimal pain, and imaging confirmed adequate ovarian position for future assisted reproductive technology (ART).

Implications:

This novel surgical method may improve outcomes for infertile patients with undescended ovaries requiring assisted reproduction.
The educational video demonstrates a potentially valuable technique for managing complex cases of ovarian malposition.
