    Background:

    • Assessing the utility of monoclonal antibody (MAb) 17-1A and its F(ab')2 fragment for intraoperative radioimmunodetection.
    • Evaluating the clinical usefulness of a hand-held gamma-detecting probe (GDP) in colorectal cancer surgery.

    Observation:

    • Radiolabeled MAb 17-1A or its F(ab')2 fragment was administered preoperatively to 18 patients with colorectal cancer.
    • Intraoperative GDP detected 75% of tumor sites with tumor-tissue ratios >= 1.5:1, averaging 2.3:1 for fragments and 3.4:1 for the whole antibody.

    Findings:

    • The GDP contributed to intraoperative decision-making in three patients by localizing undetected tumors or mapping resection margins.
    • Histologic confirmation supported the GDP's accuracy in identifying local/regional recurrence.
