Summary

Descemet stripping only (DSO) shows promising long-term results for Fuchs endothelial corneal dystrophy (FECD). Most patients maintained clear corneas and good vision for 5-6 years, with successful outcomes even after repeat procedures.

Background:

  • Fuchs endothelial corneal dystrophy (FECD) is a progressive condition affecting the cornea's inner layer.
  • Descemet stripping only (DSO) is an alternative surgical approach that removes diseased tissue without donor grafts.
  • Long-term data on DSO efficacy for FECD is limited but essential for clinical adoption.

Purpose of the Study:

  • To evaluate the 5- and 6-year outcomes of Descemet stripping only (DSO) in patients with Fuchs endothelial corneal dystrophy (FECD).
  • To assess the long-term visual acuity and corneal clarity following DSO.
  • To determine the role of DSO in the management of FECD.

Main Methods:

  • Retrospective chart review of patients diagnosed with FECD who underwent DSO between 2016 and 2017.
  • Analysis of corneal clearance, best-corrected visual acuity (BCVA), and need for subsequent procedures.

Main Results:

  • Out of 13 eyes, 12 achieved corneal clearance post-DSO.
  • At 5 years, 90% of eyes with clear corneas had a BCVA of 20/30 or better.
  • At 6 years, 87.5% of eyes maintained a VA better than 20/40.
  • Two eyes required endothelial keratoplasty (EK) but achieved good visual outcomes.

Conclusions:

  • Descemet stripping only (DSO) demonstrated a 77% success rate in maintaining clear corneas for at least 5 years in FECD patients.
  • DSO is a viable option for FECD management, offering good visual outcomes.
  • Endothelial keratoplasty (EK) remains a successful option for eyes with failed DSO.
