Background:

  • Impacted mandibular third molars (IMTMs) pose risks during extraction.
  • Understanding the proximity of the mandibular canal is vital to prevent nerve damage.

Purpose of the Study:

  • To investigate the 3D positional relationship between IMTMs and the mandibular canal using CBCT.
  • To assess the frequency and patterns of close contact between IMTMs and the mandibular canal.

Main Methods:

  • CBCT scans of 101 patients with 142 IMTMs were analyzed.
  • Three-dimensional reconstructions were used to visualize the mandibular canal relative to IMTMs.
  • Specific CBCT parameters were optimized for high-resolution imaging.

Main Results:

  • The mandibular canal was interrupted below the third molar in 59.86% of cases.
  • Close contact between the mandibular canal and IMTM roots occurred in 88.03% of cases.
  • One case showed direct insertion of the third molar into the mandibular canal.

Conclusions:

  • CBCT provides a dynamic, 3D understanding of the IMTM-mandibular canal relationship.
  • This imaging modality offers high clinical value for surgical planning.
