Summary

Castleman disease (CD) is a rare lymphoproliferative disorder. Unicentric CD (UCD) shows better outcomes with surgery, while multicentric CD (MCD) requires chemotherapy, with a 91% 3-year survival rate observed.

Area of Science:

  • Hematology
  • Oncology
  • Rare Diseases

Background:

  • Castleman disease (CD) encompasses rare, heterogeneous lymphoproliferative disorders marked by enlarged lymph nodes.
  • CD is broadly classified into unicentric (UCD) and multicentric (MCD) forms.

Purpose of the Study:

  • To retrospectively analyze the clinical data of 11 patients diagnosed with Castleman disease between 2017 and 2022.
  • To evaluate treatment strategies and outcomes for both UCD and MCD subtypes.

Main Methods:

  • Retrospective review of patient data from a tertiary cancer center.
  • Analysis of demographic, histological, treatment, and survival data for 11 CD patients (7 UCD, 4 MCD).

Main Results:

  • The hyaline-vascular variant was the predominant histology in both UCD and MCD.
  • UCD patients treated with surgery achieved high response rates (6 complete, 1 partial).
  • MCD patients received systemic therapies including rituximab and chemotherapy (RCHOP/CHOP), with varied responses and one fatality.

Conclusions:

  • Castleman disease is a rare condition, with UCD being more prevalent and having a favorable prognosis.
  • Surgical excision is the primary treatment for UCD, yielding excellent outcomes.
  • MCD necessitates systemic treatment, often involving combination chemotherapy, and is associated with a lower survival rate compared to UCD.
