Heart rate turbulence (HRT) is a marker of autonomic cardiac regulation. Severely abnormal HRT in acute ischemic stroke patients is linked to greater disability and cardiac issues, but not mortality.

- Severely abnormal HRT was associated with increased disability (higher mRS) at 3 months (aOR: 2.9) and new atrial fibrillation (aOR: 3.5).
- Severely abnormal HRT was also linked to MRI-detected myocardial fibrosis (aOR: 5.8).
- No significant association was found between HRT categories and 1-year mortality; abnormal HRT was not linked to any analyzed outcomes.
Conclusions:
- Severely abnormal HRT is associated with increased disability and cardiac comorbidities in acute ischemic stroke patients.
- HRT may help identify patients needing further monitoring for atrial fibrillation and cardiac imaging.
