    Summary

    Pattern reversal retinal potential (PRRP) reductions occur in some ocular hypertension (OHT) patients at risk for primary open-angle glaucoma (POAG). However, many OHT patients, especially those at low risk, show normal PRRPs, indicating varied disease progression.

    Background:

    • The human-pattern reversal retinal potential (PRRP) measures neural activity in the retina.
    • Reductions in PRRP amplitude are linked to visual diseases affecting retinal ganglion cells and the optic nerve, such as primary open-angle glaucoma (POAG).

    Purpose of the Study:

    • To investigate if ocular hypertension (OHT) patients, who are at risk for POAG, also exhibit PRRP amplitude reductions.
    • To differentiate PRRP responses in OHT patients based on their risk level for developing POAG.

    Main Methods:

    • Elicited PRRPs using high-contrast, phase-alternating checkerboard patterns in patients with POAG (n=12), OHT (n=24), and age-matched visual normals (n=11).
    • Categorized OHT patients into high and low risk groups for POAG development.

    Main Results:

    • PRRP amplitude reductions were observed in some OHT patients, similar to those seen in POAG patients.
    • A significant number of OHT patients, particularly those at low risk for POAG, did not show abnormal PRRP amplitudes.

    Conclusions:

    • PRRP testing can identify potential early neural dysfunction in some OHT patients at risk for POAG.
    • PRRP abnormalities are not universal in OHT patients, suggesting heterogeneous disease pathways or progression rates.
