    Summary

    Magnetic resonance (MR) imaging shows high accuracy in staging endometrial cancer and assessing myometrial invasion. However, MR imaging cannot differentiate tumor from benign conditions, making histology essential for diagnosis.

    Background:

    • Endometrial carcinoma is a common gynecologic malignancy.
    • Accurate staging is crucial for effective treatment planning.

    Purpose of the Study:

    • To prospectively evaluate the diagnostic potential of magnetic resonance (MR) imaging for endometrial carcinoma detection and staging.
    • To compare MR imaging findings with surgical-pathologic results.

    Main Methods:

    • Prospective evaluation of 51 patients with suspected endometrial carcinoma.
    • Comparison of MR imaging findings with post-hysterectomy surgical-pathologic staging and lymph node sampling.

    Main Results:

    • MR imaging detected an endometrial abnormality in 84% of patients.
    • Overall accuracy for staging endometrial carcinoma was 92%, and for myometrial invasion was 82%.
    • MR imaging could not differentiate endometrial carcinoma from adenomatous hyperplasia or blood clots; lymphadenopathy and distant metastases detection were suboptimal.

    Conclusions:

    • Histologic diagnosis remains essential for endometrial carcinoma detection due to MR imaging's lack of specificity.
    • MR imaging is a valuable tool for assessing the extent of endometrial carcinoma, particularly myometrial invasion.
    • Further improvements are needed for MR imaging in detecting lymph node and distant metastases.
