    Area of Science:

    • Neurosurgery
    • Medical Devices
    • Neurosurgical Techniques

    Background:

    • Accurate ventricular access is crucial for managing various neurological conditions.
    • Traditional methods for ventricular catheter placement can be associated with complications.

    Purpose of the Study:

    • To evaluate the safety, efficacy, and accuracy of a novel low-profile device, the Device for Intraventricular Entry (DIVE) guide, for ventricular access.
    • To assess the DIVE guide's performance in a prospective study at a large academic center.

    Main Methods:

    • A prospective study involving 50 patients undergoing external ventricular drainage or shunt placement.
    • The Device for Intraventricular Entry (DIVE) guide was used for assisted catheter placement.
    • Primary outcome: postprocedural CT confirmation of catheter tip location. Secondary outcomes: number of passes, hemorrhages, infections.

    Main Results:

    • Successful ventricular cannulation in 100% of 50 patients.
    • 92% of patients achieved optimal catheter tip placement (Kakarla grade 1).
    • Average of 1.06 passes per procedure with no clinically significant hemorrhages or infections.

    Conclusions:

    • The DIVE guide facilitates safe and effective ventricular access.
    • High accuracy and complication-free performance demonstrated in this prospective study.
    • The DIVE guide represents a promising advancement in neurosurgical procedures requiring ventricular access.
