    Burn injury alters drug pharmacokinetics. Meperidine clearance was lower than expected in burn patients, suggesting reduced hepatic drug metabolism despite increased blood flow.

    Background:

    • Limited data exists on pharmacokinetics of drugs cleared by the liver in burn patients.
    • Burn injury is known to increase hepatic blood flow, potentially affecting drug clearance.

    Purpose of the Study:

    • To investigate the pharmacokinetics of intravenous meperidine in burn patients.
    • To test the hypothesis that meperidine clearance increases post-burn due to elevated hepatic blood flow.

    Main Methods:

    • Studied intravenous meperidine pharmacokinetics in 11 burn patients acutely (1 week post-burn) and 5 patients convalescently (6 weeks post-burn).
    • Measured meperidine steady-state distribution volume and clearance.

    Main Results:

    • Meperidine steady-state distribution volume was approximately half that reported for healthy subjects in both acute and convalescent phases.
    • Meperidine clearance (acute: 420 ml/min; convalescent: 600 ml/min) was lower than anticipated, despite increased hepatic blood flow in burn patients.
