    Background:

    • Historically, mandatory surgical exploration for all platysma-penetrating neck wounds was standard practice.
    • Recent evidence suggests selective exploration may be a safe and viable alternative.

    Purpose of the Study:

    • To evaluate the safety and efficacy of a selective exploration policy for penetrating neck injuries.
    • To compare outcomes between immediate surgical exploration and observation.

    Main Methods:

    • Retrospective review of 136 consecutive patients with penetrating neck wounds treated between 1972 and 1984.
    • Patients were managed with either immediate exploration or selective observation based on clinical assessment and diagnostic studies.
    • Diagnostic studies included arch aortography and esophagography where indicated.
