Background:

  • The World Health Organization (WHO) revised its annual mean nitrogen dioxide (NO2) standard to 10 μg/m³ due to evidence linking NO2 exposure to excess mortality.
  • Previous studies indicated a correlation between ambient NO2 and increased mortality, necessitating updated analyses.

Approach:

  • A comprehensive meta-analysis was conducted, systematically searching PubMed, Web of Science, and Scopus for studies published up to July 1, 2022.
  • Eleven new studies were incorporated into the analysis, alongside 45 previously identified studies, totaling 56 included articles.
  • Random effects models were used to calculate pooled risk ratios (RR) and 95% confidence intervals (CIs), with sensitivity and publication bias analyses performed.

Key Points:

  • Long-term exposure to ambient NO2 is associated with increased all-cause mortality (RR: 1.03, 95% CI: 1.02-1.05), cardiovascular mortality (RR: 1.07, 95% CI: 1.04-1.10), and respiratory mortality (RR: 1.03, 95% CI: 1.02-1.05) per 10 μg/m³ increase.
  • Significant heterogeneity was observed across studies (I² = 84%-96%).
  • Subgroup analyses indicated particularly elevated risks in Asia and Oceania (p < 0.05), with aggregated mortality risks slightly exceeding previous estimates.

Conclusions:

  • The findings underscore the public health threat posed by ambient NO2, reinforcing the need to meet stricter air quality standards.
  • Further research should focus on developing patient cohorts, conducting age-refined analyses, and utilizing high-spatiotemporal-resolution data to reduce study heterogeneity.
  • Establishing additional cohorts in Asia and Oceania is crucial to strengthen evidence from these regions.
