    Summary

    This study reports the first known case of Group D2 pyelonephritis in a 72-year-old man. Previously, Group D2 organisms were only linked to alkaline-encrusted cystitis and struvite stones.

    • Diagnostic investigations identified Group D2 organisms as the causative agent.

    Findings:

    • This case represents the first documented instance of Group D2 pyelonephritis.
    • The clinical presentation and microbiological findings challenge existing associations of Group D2 organisms.

    Implications:

    • This finding expands the known clinical spectrum of Group D2 organism infections.
    • Further research is warranted to understand the pathogenicity and clinical significance of Group D2 pyelonephritis.
