Massive hemoptysis from bronchial hemangioma is rare and often fatal. Surgical resection is recommended for complex cases unresponsive to bronchial artery embolization (BAE).
Area of Science:
- Pulmonology
- Vascular Surgery
- Interventional Radiology
Background:
- Bronchial hemangioma is a rare condition in adults.
- Massive hemoptysis from diffuse vascular proliferation can be fatal.
Observation:
- A 29-year-old woman experienced recurrent massive hemoptysis.
- Repeated interventional embolization was ineffective.
- Right upper lung lobectomy with extracorporeal membrane oxygenation support was performed.
Findings:
- The patient recovered and had no hemoptysis recurrence at 4-year follow-up.
- Surgical resection was successful in managing intractable hemoptysis.
Implications:
- Early surgical intervention may be crucial for bronchial hemangiomas with arteriovenous fistulae.
- Bronchial artery embolization (BAE) may be insufficient for diffuse vascular lesions.
- Aggressive management is necessary for life-threatening hemoptysis due to bronchial hemangioma.
