    Background:

    • Leiomyosarcoma of the kidney capsule is a rare renal tumor.
    • Surgical resection (nephrectomy) is the primary treatment modality.

    Observation:

    • A 71-year-old woman presented with a kidney capsule leiomyosarcoma.
    • The tumor showed no invasion and few mitoses but recurred 13 months post-nephrectomy.

    Findings:

    • Combination chemotherapy induced a full subjective and near-complete objective tumor regression lasting 8 months.
    • This suggests potential efficacy of chemotherapy in managing recurrent renal capsule leiomyosarcoma.

    Implications:

    • Chemotherapy may be a viable option for managing recurrent kidney capsule leiomyosarcoma.
    • Further research into systemic therapies for this rare entity is warranted.
