    Background:

    • A 25-year-old female presented with a history of syncopal attacks during strenuous physical activity.
    • She also reported urticaria, pruritus, and numbness with prolonged exertion.
    • Hospitalization revealed that treadmill stress tests induced bronchial spasm, urticaria, and hypotension.

    Findings:

    • The case suggests a potential link between physical exertion and anaphylactic episodes.
    • The co-occurrence of urticaria and anaphylaxis points towards the release of vasoactive substances.
