Sleep Apnea01:21

Sleep Apnea

160
Sleep apnea is a condition where breathing stops intermittently during sleep, often leading to significant health issues. Each episode can last from 10 to 20 seconds or more and is frequently accompanied by a brief arousal from sleep. This disturbance, largely unnoticed by the individual, can lead to severe daytime fatigue. Commonly, individuals seek help after being informed by their partners about loud snoring and noticeable breathing pauses during sleep.
The condition is more prevalent among...

Acute Respiratory Failure-III01:30

Acute Respiratory Failure-III

187
Hypercapnic respiratory failure, also known as Type 2 or ventilatory respiratory failure, is a severe condition characterized by the body's inability to effectively remove carbon dioxide (CO2) from the bloodstream. It leads to an arterial CO2 pressure (PaCO2) exceeding 45 mmHg and a blood pH above 7.35. This situation indicates that the body's ventilatory demand, or the ventilation needed to maintain normal PaCO2 levels, surpasses its supply or the maximum gas flow achievable without...

Oxygen Delivering System III: Tracheostomy and T-piece01:23

Oxygen Delivering System III: Tracheostomy and T-piece

1.1K
Oxygen delivery is critical in clinical care, especially for patients with respiratory disorders or those undergoing surgical procedures. Various systems, such as tracheostomy and the T-piece, deliver oxygen to the lungs, ensuring adequate arterial oxygenation.
Tracheostomy
A tracheostomy is a surgically created opening (stoma) in the anterior part of the trachea. It is used to establish a patient airway, bypass an upper airway obstruction, simplify the removal of secretions, permit long-term...

Oxygen Delivering System II: Venturi Mask and Transtracheal Oxygen

631
Oxygen therapy is a pivotal aspect of medical care, particularly for patients with respiratory ailments. Two prominent oxygen-delivering systems include the Venturi mask and the transtracheal oxygen catheter.
Venturi Mask
The Venturi mask, named after the Venturi effect, is designed to deliver precise oxygen concentrations. It consists of a large tube with an oxygen inlet that narrows down, causing a pressure drop that pulls air in through adjustable side ports. The mask is a lightweight,...

Angle Closure Glaucoma: Treatment01:28

Angle Closure Glaucoma: Treatment

528
Angle-closure glaucoma, or closed-angle glaucoma, is an eye condition where the iris bulges out and blocks the iridocorneal angle, resulting in a buildup of aqueous humor and increased intraocular pressure. Immediate medical attention is necessary due to the sudden onset of symptoms. The treatment for angle-closure glaucoma includes short-term and long-term approaches. Short-term treatment involves using eye drops like pilocarpine to lower intraocular pressure by increasing aqueous humor...

Summary

Obstructive sleep apnea (OSA) significantly increases the risk of revision surgery after ptosis repair. Patients with OSA had a 1.7 times higher chance of needing a repeat procedure for eyelid issues.

Area of Science:

  • Ophthalmology
  • Sleep Medicine
  • Surgical Outcomes

Background:

  • Ptosis repair aims to correct eyelid drooping.
  • Obstructive sleep apnea (OSA) is a common condition affecting breathing during sleep.
  • The impact of OSA on ptosis repair outcomes is not well-established.

Purpose of the Study:

  • To investigate the effect of Obstructive sleep apnea (OSA) on the outcomes of ptosis repair.
  • To determine if patients with OSA have a higher rate of reoperation after ptosis repair.

Main Methods:

  • Retrospective cohort study of patients over 18 undergoing ptosis repair (2018-2021).
  • Surgical failure defined as asymmetry or unsatisfactory eyelid height requiring revision within 1 year.
  • Comparison of outcomes between patients with and without OSA.

Main Results:

  • 577 patients included; 20.5% of OSA patients experienced surgical failure vs. 13.1% without OSA (p=0.02).
  • OSA patients had a 1.70 times higher risk of revision surgery (95% CI: 1.06-2.07).
  • Logistic regression confirmed OSA significantly associated with ptosis revision (p=0.007) after adjusting for age and sex.

Conclusions:

  • Obstructive sleep apnea (OSA) is associated with an increased risk of surgical failure and revision surgery following blepharoptosis repair.
  • While OSA is a risk factor, it is not the sole determinant of revision surgery necessity.
  • Revision surgeries in this cohort resulted in satisfactory outcomes for all patients.
