A direct, high-flow carotid-cavernous fistula following head trauma caused right eye proptosis and ophthalmoplegia. Endovascular occlusion successfully resolved the patient's symptoms.

- An endovascular procedure was performed to treat the carotid-cavernous fistula.
- Successful occlusion of the fistula was achieved.
Implications:
- Endovascular treatment offers an effective therapeutic option for post-traumatic carotid-cavernous fistulas.
- Prompt diagnosis and intervention can lead to significant symptom resolution and prevent further complications.
