Background:

  • Upper urinary tract (UUT) obstruction in children can arise from complex anatomical variations.
  • Laparoscopic techniques offer a minimally invasive approach to address these challenging cases.

Purpose of the Study:

  • To review the outcomes of primary laparoscopic UUT reconstructions for various anatomical anomalies.
  • To evaluate the efficacy and safety of different laparoscopic strategies in pediatric patients.

Main Methods:

  • Retrospective review of 214 laparoscopic UUT reconstructions from May 2012 to May 2021.
  • Included anomalies: malrotated kidney (MRK), horseshoe kidney (HSK), duplex kidney (DK), intrarenal pelvis (IRP), and mid-ureteric stenosis (MUS).
  • Success defined by symptom resolution, improved renal pelvic diameter (APD), and drainage on Mag3 scans.

Main Results:

  • 37 cases involved complex anomalies, with a 94.5% success rate (35/37).
  • Significant improvement in hydronephrosis (median APD reduced from 27mm to 8mm) and renal drainage was observed.
  • Minor complications included UTIs and stent-related issues; major complications were rare.

Conclusions:

  • Laparoscopic transperitoneal surgery is a versatile and effective approach for UUT obstruction in pediatric patients with anatomical variants.
  • Excellent outcomes and high success rates support the use of laparoscopic strategies for these complex cases.
