Area of Science:

  • Urology
  • Surgical Oncology
  • Robotic Surgery

Background:

  • The lateral approach to radical prostatectomy enables a buttonhole incision for direct access to the seminal vesicle.
  • This technique allows for complete preservation of the anterior pubovesical complex.

Purpose of the Study:

  • To evaluate the reproducibility and outcomes of robotic radical prostatectomy using the lateral approach.
  • To assess intraoperative, postoperative, oncological, and functional parameters of this surgical technique.

Main Methods:

  • Analysis of 513 patients undergoing robotic radical prostatectomy by lateral approach between January 2015 and March 2021.
  • Comparison of oncological and functional results between two experienced surgeons performing the procedure.

Main Results:

  • No significant difference in positive surgical margins (32.87% vs 37.9%) or significant margins > 2 mm (5.88% vs 7.58%) between surgeons.
  • High rates of immediate continence (86% vs 85%) and sexual potency at one year (73% vs 72%) were observed.
  • Complication rates were similar for both surgeons.

Conclusions:

  • Robotic radical prostatectomy via the lateral approach is a reproducible technique.
  • The method demonstrates favorable oncological and functional results.
  • Preservation of the anterior pubovesical complex is a key feature of this approach.
