Insights
Pediatricians widely use melatonin (MLT) for healthy children
Area of Science:
- Pediatrics
- Pharmacology
- Sleep Medicine
Background:
- Melatonin (MLT) is frequently used by pediatricians for sleep issues.
- Evidence on MLT use in healthy children is limited.
- This study examines MLT prescription patterns in Italian pediatricians for healthy children with chronic insomnia.
Purpose of the Study:
- To describe the current practices of Italian pediatricians regarding melatonin use in healthy children with chronic insomnia.
- To gather data on age ranges, dosages, administration timing, treatment duration, and associated therapies.
- To assess perceived efficacy and side effects of melatonin in this population.
Main Methods:
- A cross-sectional survey was conducted among Italian pediatricians from June to November 2022.
- Data collected included MLT dosage, administration, duration, co-treatments, efficacy, and side effects.
- Statistical analysis, including chi-square tests, compared responses based on sleep medicine training.
Main Results:
- 97.4% of 428 surveyed pediatricians reported using MLT.
- MLT was commonly prescribed for ages 1-2 years (87.3%), 2-5 years (62.1%), and 10-18 years (42.5%).
- Most prescribed 1 mg/day, 30 minutes before bed, for 1-3 months, often with sleep hygiene. Efficacy was high, with few mild side effects.
Conclusions:
- Melatonin is extensively prescribed by Italian pediatricians for healthy children's insomnia.
- Significant variation exists in MLT usage protocols.
- There is a clear need for evidence-based guidelines to standardize MLT use in healthy pediatric populations.
Objectives:
Although melatonin (MLT) is the molecule most used by pediatricians for sleep problems, scarce evidence exists on its use in healthy pediatric population. The objective of this study was to describe MLT use by Italian pediatricians in healthy children with chronic insomnia.
Study Design:
A cross-sectional open survey was administered to Italian pediatricians, between June and November 2022, collecting information about their use of MLT in healthy children: age range of patients, dosages used, time of administration, duration of treatment, association with other treatments, perceived efficacy, and side effects. Data were reported as frequencies with their respective 95% confidence intervals. Chi-square statistics assessed significant differences between pediatricians who had training in pediatric sleep and those who did not.
Results:
Among 428 respondents, 97.4% of pediatricians used MLT; 87.3% of them prescribed MLT in children aged 1-2 years, 62.1% in 2-5 years and 42.5% in 10-18 years. 84.9% of them suggested to take MLT 30 min before bedtime. 37.9% indicated to continue treatment for one month, 30.2% for 2-3 months. 74.1% of pediatricians usually prescribed MLT 1 mg/day. The most frequent treatment associated with MLT was sleep hygiene (85.4%). Almost all pediatricians found MLT effective in reducing difficulties falling asleep. Only 3.2% of them reported mild side effects.
Conclusions:
MLT is widely prescribed by Italian pediatricians, but no consensus exists about its use in typically developing children. There is a need for clear guidelines to optimize the use of MLT in healthy children.

Sedatives and Hypnotics Drugs: Miscellaneous Agents
Melatonin congeners like ramelteon (Rozerem) and tasimelteon (Hetlioz) selectively bind to melatonin receptors (MT1 and MT2) and thus mimic the actions of melatonin, a hormone that regulates sleep-wake cycles. Tasimelteon is primarily used for non-24-hour sleep-wake disorder, common in blind patients. They are also used to treat conditions like insomnia...

Sedatives and Hypnotics: Overview
Sedative-hypnotics are categorized into barbiturates, benzodiazepines (BZDs), and non-benzodiazepines or Z-drugs. These drugs work by suppressing central nervous system activity, and this suppression is dose-dependent. Older sedative medications, like barbiturates, follow a linear curve in...
