    Summary

    This case report details a rare late recurrence of endometrial carcinoma over 26 years post-diagnosis. It highlights that even favorable prognostic factors do not preclude exceptionally delayed cancer recurrence.

    Background:

    • Endometrial carcinoma recurrence typically occurs within the first few years after initial treatment.
    • Late recurrences, defined as occurring more than 5 years post-treatment, are uncommon.

    Observation:

    • A 47-year-old patient with Stage IA, FIGO grade 2 adenoacanthoma underwent treatment with radium implant and radical hysterectomy.
    • The initial tumor was confined to the corpus with superficial myometrial invasion.

    Findings:

    • The patient experienced an isolated lower vaginal recurrence 26.5 years after her initial diagnosis and treatment.
    • Histological examination confirmed the recurrence was identical to the primary tumor.

    Implications:

    • This case underscores the possibility of exceptionally late recurrences of endometrial cancer, even in patients with favorable prognostic indicators.
    • Clinicians should maintain awareness of late recurrence potential in long-term survivors of endometrial carcinoma.
