Summary

Angiotensin receptor blockers (sartans) can cause lung toxicity. This case shows drug-induced interstitial lung disease from two different sartans in the same patient over a decade.

Observation:

  • A 60-year-old male experienced interstitial lung disease symptoms, including dyspnea and hemoptysis, attributed to eprosartan in 2012.
  • Symptoms resolved completely upon discontinuation of eprosartan.
  • Ten years later, the patient developed similar symptoms and radiographic findings after initiating valsartan, another sartan.

Findings:

  • The patient experienced two distinct episodes of drug-induced interstitial lung disease.
  • Both episodes were linked to different sartans (eprosartan and valsartan).
  • The clinical presentation and chest CT imaging were comparable between the two events.

Implications:

  • This case highlights the potential for sartans to cause interstitial lung disease.
  • It suggests that patients may react to multiple drugs within the sartan class.
  • Further investigation into sartan-induced lung toxicity is warranted.
