Summary

Background:

  • Migraine attacks are associated with visual symptoms, but the retina's role remains unclear.
  • Optical coherence tomography angiography (OCTA) offers high-resolution, non-invasive visualization of retinal microvasculature.
  • This study investigates retinal vascular changes in migraine patients during and between attacks using OCTA.

Purpose of the Study:

  • To assess structural and functional changes in retinal vasculature during and between migraine attacks.
  • To compare retinal vascular features in patients with migraine with aura (MA) and migraine without aura (MO) against healthy controls (HCs).
  • To explore the potential of OCTA-derived retinal vascular signatures as migraine biomarkers.

Main Methods:

  • Prospective cohort study including 37 MA, 30 MO, and 20 HCs.
  • Macular OCTA scans were performed interictally for all participants.
  • OCTA captured scans during and between attacks in a subset of patients and controls, analyzing foveal avascular zone morphology, vessel flux index (VFI), and vessel area density.

Main Results:

  • Parafoveal VFI significantly decreased during migraine attacks in both MA and MO groups compared to HCs.
  • Interictally, MA patients showed reduced foveal perfusion and increased foveal avascular zone circularity compared to MO patients.
  • Patients with unilateral headaches exhibited interictal retinal perfusion asymmetry between eyes ipsilateral and contralateral to pain.

Conclusions:

  • Migraine attacks are associated with reduced parafoveal retinal perfusion.
  • Distinct interictal retinal vascular changes are present in migraine patients, differing between MA and MO.
  • OCTA may identify retinal vascular biomarkers relevant to migraine pathophysiology and diagnosis.
