Background:

Endoscopic and laparoscopic surgery is emerging for treating appendiceal base tumors.
This combined approach offers a minimally invasive alternative to traditional surgery.
It holds potential for improving safety in surgical treatment of appendiceal neoplasms.

Observation:

A patient with a large ileocecal adenoma involving the appendiceal orifice initially underwent attempted endoscopic resection.
Due to intraabdominal adhesions, a laparoscopic adhesiolysis was performed, followed by endoscopic appendectomy.
Endoscopic purse-string sutures were used for closure, with dual drainage tubes placed.

Findings:

The patient experienced an uncomplicated recovery after the appendectomy.
One-year follow-up endoscopy confirmed excellent closure of the surgical site.
No adverse complications were reported post-procedure.

Implications:

The integration of internal medicine and surgery is crucial for medical advancement.
Laparoscopic and endoscopic combined surgery fosters interdisciplinary cooperation, improving patient outcomes.
This technique shows promise, warranting further clinical data for validation.
