    Observation:

    • Bone marrow biopsy revealed Gaucher-like cells alongside plasma cells in a multiple myeloma patient.
    • Microscopic examination showed large cells with specific staining characteristics.
    • Electron microscopy revealed crystals within plasma cells and macrophages.

    Findings:

    • Cells exhibited histochemical properties of Gaucher cells (PAS positive, diastase resistant).
    • Immunofluorescence showed these cells contained kappa and gamma immunoglobulin chains.
    • Electron microscopy confirmed macrophages engorged with immunoglobulin, not glucocerebrosidase deficiency products.

    Implications:

    • These pseudo-Gaucher cells are distinct from true Gaucher cells.
