Summary

The effectiveness of corticosteroids in preventing postherpetic neuralgia (PHN) remains uncertain due to very low-certainty evidence. Corticosteroids show little difference in adverse events for acute herpes zoster patients.

Background:

  • Postherpetic neuralgia (PHN) is a debilitating complication of herpes zoster.
  • Corticosteroids possess anti-inflammatory properties with potential therapeutic benefits.
  • This review is an updated analysis of corticosteroid efficacy for PHN prevention.

Purpose of the Study:

  • To evaluate the benefits and harms of corticosteroids in preventing PHN.
  • To synthesize evidence from randomized controlled trials (RCTs) on corticosteroid use for PHN prevention.

Main Methods:

  • Comprehensive search of multiple databases and trial registers up to June 2022.
  • Inclusion of RCTs comparing corticosteroids (oral, intramuscular, intravenous) with placebo or no treatment for acute herpes zoster.
  • Independent data extraction, risk of bias assessment, and certainty of evidence evaluation by two reviewers.

Main Results:

  • Five RCTs with 787 participants were included; no new studies were identified for this update.
  • Evidence is very uncertain regarding the effect of oral corticosteroids on preventing PHN at six months (RR 0.95, 95% CI 0.45 to 1.99).
  • No significant differences in serious or non-serious adverse events were observed between corticosteroid and placebo groups.

Conclusions:

  • The current evidence is insufficient to determine the efficacy of oral corticosteroids in preventing PHN.
  • Corticosteroids appear to have minimal impact on adverse events in acute herpes zoster.
  • Future research should incorporate long-term follow-up and quality of life measures to assess corticosteroid effects on PHN.
