Summary

Urothelial malignancy screening after augmentation cystoplasty (AC) can be challenging. Urine fluorescence in situ hybridization (FISH) demonstrated higher specificity than conventional cytology (CC) for detecting bladder cancer in this study.

Background:

  • Malignancy occurs in up to 5.5% of patients following augmentation cystoplasty (AC).
  • Effective screening methods for bladder cancer post-AC are crucial for early detection and management.

Purpose of the Study:

  • To evaluate the utility of urine fluorescence in situ hybridization (FISH) for screening bladder malignancy after augmentation cystoplasty.
  • To compare the diagnostic performance of FISH with conventional cytology (CC).

Main Methods:

  • Prospective study including 36 patients 10 years post-ileal AC.
  • Urine FISH and conventional cytology (CC) performed on a subset of patients.
  • Cystoscopy with random biopsies conducted on 32 patients, blinded to cytology results.

Main Results:

  • Two patients (5.6%) were diagnosed with adenocarcinoma via biopsy.
  • FISH analysis detected malignancy in 3/24 (12.5%) patients, with 100% sensitivity and 95% specificity.
  • Conventional cytology detected malignancy in 2/4 patients, with 100% sensitivity and 91.6% specificity.

Conclusions:

  • Urine FISH shows promise as a screening tool for malignancy after augmentation cystoplasty.
  • FISH exhibited higher specificity compared to conventional cytology in this patient cohort.
  • Further research with larger cohorts is warranted to confirm these findings.
