Background:

Patient adherence to rehabilitation regimens significantly impacts recovery outcomes post-stroke.
Developing reliable measurement tools is crucial for evaluating and improving adherence in stroke rehabilitation programs.

Purpose of the Study:

To design and validate a psychometric scale for measuring adherence to rehabilitation regimens in Iranian patients post-stroke.
To establish the face, content, and construct validity, along with the reliability, of the new scale.

Main Methods:

A sequential exploratory mixed-methods design was employed, involving qualitative (n=20) and quantitative (n=198) phases.
Psychometric properties, including validity and reliability, were rigorously assessed using established statistical methods.

Main Results:

The Adherence to Rehabilitation Regimen Scale (ARRS) was developed with 26 items across four factors: physical exercises, following regimens, daily living activities, and psychological follow-up.
The scale exhibited excellent internal consistency (Cronbach's alpha = 0.96) and reliability (ICC = 0.99).

Conclusions:

The ARRS is a valid and reliable instrument for measuring adherence to rehabilitation regimens in post-stroke patients.
This scale can be a valuable tool for clinicians and researchers aiming to manage and enhance adherence in stroke rehabilitation programs.
